$50 Billion Rural Health Transformation Program (RHTP)
Established under the One Big Beautiful Bill (P.L. 119-21), the RHTP is the largest rural health investment in U.S. history — $10 billion per year from FY 2026 through FY 2030, awarded to all 50 states. Every state received a first-year award averaging $200 million. Funds flow from CMS through state agencies, which must implement approved Rural Health Transformation Plans across five CMS strategic goals. TSCTI supports state agencies as an implementation partner across all five goals.
